Licensed Practical Nurse (LPN) – Per Diem (In-Home Care)
Pay: $35 per hour
Schedule: 12-hour shifts, 3–4 days per week
Location: Various client homes throughout the area
About the Role
We are seeking reliable and compassionate Licensed Practical Nurses (LPNs) to provide in-home care for patients requiring ongoing support. This is a per diem position—ideal for nurses who want flexible scheduling while delivering high-quality care.
Responsibilities
Provide direct patient care in the home
Monitor patient condition and document clinical data
Administer medications and treatments as ordered
Assist with activities of daily living
Communicate effectively with family members and care teams
Maintain accurate and timely clinical notes
Requirements
Active New York State LPN license
Minimum 1 year of nursing experience (home care preferred)
Ability to work 12-hour shifts
Reliable transportation
Strong clinical judgment and communication skills
Compensation
$35 per hour
